Initial Contact

Once you contact us, our friendly team will reach out to you to schedule an initial meeting. We will discuss the purpose of the meeting and gather some basic information to ensure we are well-prepared to assist you effectively.

2. Introduction and Information Exchange
|
During our first meeting we will introduce ourselves and provide an overview of our services. We can meet in-person, over the phone or video call. We will explain how the NDIS works and answer any questions or concerns you may have. You can share information about your disability, your goals, and any existing supports you already have in place.

3. Individualised Support Planning
|
To better understand your specific needs, our team will conduct a
comprehensive assessment. This is a conversation to gain insight into
your daily living requirements, mobility, communication preferences,
health conditions, social interactions, and any other relevant aspects.
We will ensure a comfortable and open environment. For everyone to be on
the same page, we encourage you to express your needs, desires, and
aspirations. Using this information, our experienced team will work closely with you to develop an individualised support plan. This plan will outline the goals you want to achieve and the supports required to reach those goals. It will reflect the things you told us during the assessment.

4. Coordinated Support Network
|
If you are receiving support coordination, we will work with you to establish relationships with a network of support providers who can assist you in various areas. We will find services to suit you. This may include support workers, therapists, allied or other healthcare professionals, or any other services that are necessary for you. We will make contact, keep open lines of communication, ensure service agreements are in place, and monitor whether everything is working well.

5. Ongoing Review and Adjustments
|
We understand that needs and circumstances can change over time. We conduct regular reviews of your support plan to ensure it is fit for purpose. We are only ever a phone call or email away, if you have any questions or concerns. We talk to you about how we can help build capacity.
